Testes com animais, seres humanos e avaliações técnicas mostraram eficiência do respirador

Inspire, ventilador pulmonar emergencial desenvolvido na USP pode ser produzido em até duas horas, sendo mais barato dos que os aparelhos disponíveis no mercado; enquanto ventiladores pulmonares convencionais custam, em média, R$ 15 mil, valor do Inspire é de R$ 1 mil passou pelas etapas finais de testes. 

Como próximos passos, os documentos relativos ao projeto serão enviados aos órgãos competentes, inclusive à Agência Nacional de Vigilância Sanitária (Anvisa).

O Inspire foi desenvolvido pela equipe do professor da Poli, Raul González Lima. 

Animais e seres humanos

A Poli informou que nos últimos dias dias 17, 18 e 19 de abril foram realizados estudos com pacientes humanos, seguindo os trâmites da Comissão Nacional de Ética em Pesquisa. O ensaio, sob a coordenação do professor José Otávio Auler Junior, teve também a colaboração da professora Filomena  Galas, da Faculdade de Medicina da USP (FMUSP), e do fisioterapeuta Alcino Costa Leme. Os testes foram realizados com quatro pacientes, nas dependências do Instituto do Coração (Incor) do Hospital das Clínicas (HC) da FMUSP, e o respirador foi considerado aprovado em todos os modos de uso. Não houve nenhum problema com os pacientes ventilados.

Antes disso, em 13 e 14 de abril, também havia sido realizado um estudo com animais, coordenado pela professora Denise Fantoni e com auxílio da professora Aline Ambrósio, ambas da Faculdade de Medicina Veterinária e Zootecnia (FMVZ) da USP. Os ensaios foram realizados no Laboratório de Investigações Médicas 8 (LIM8), da FMUSP. O equipamento foi testado em dois animais e considerado aprovado.

O aparelho desenvolvido pelos pesquisadores da Poli foi registrado com uma licença open source, que permite a qualquer pessoa ou empresa acessar o protocolo de manufatura e fabricá-lo, bastando, para tanto, obter uma autorização da Anvisa. Para mais informações sobre a licença, clique aqui.

 Provas técnicas

Uma das avaliações técnicas que antecederam os ensaios clínicos foi realizada no último dia 12 de abril, com a colaboração do Laboratório de Diagnóstico Avançado de Combustão do Fapesp Shell Research Centre for Gas Innovation (RCGI), sediado na Poli. O laboratório coordenado pelo professor Guenther Krieger Filho tem como objetivo original analisar reações de combustão com técnicas a laser, mas se juntou aos pesquisadores nos esforços para conter a pandemia.

Era necessário testar o protótipo para conhecer as vazões e as concentrações de oxigênio que o equipamento consegue oferecer aos pacientes, nas diferentes frequências que simulam o processo de respiração pelo pulmão humano, explica Krieger Filho. O laboratório tem um analisador de gases e um medidor de vazão de gases, e por isso ofereceu ajuda, já que havia urgência em fazer esses testes.

Segundo Krieger Filho, na corrida contra o tempo que caracteriza esse período de pandemia, o mais importante é conseguir prover ajuda real em tempo hábil. A equipe do Inspire tinha pressa e o laboratório estava lá para ajudá-los, ressalta. Neste caso, provavelmente não haveria a opção de esperar a situação ideal para testar em outro laboratório, também ideal – no sentido de mais adaptado ao projeto. Ficamos felizes de poder ajudar. É uma contribuição valiosa do RCGI nesses tempos difíceis que atravessamos.

As linhas de gases (oxigênio, nitrogênio e dióxido de carbono), bem como medidores e analisadores de gases do laboratório para desenhar um teste que conseguisse indicar o teor de oxigênio entregue pelos respiradores. Segundo o professor Krieger Filho, como os medidores do laboratório são voltados para a análise de processos de combustão em motores veiculares e queimadores industriais, foi preciso usar a criatividade para adaptar às condições da estrutura aos testes do Inspire no prazo exíguo que a tarefa exigia.

Por exemplo, o medidor de gases de que dispomos só mede a presença de oxigênio na mistura gasosa até 31%; é o que ele permite. Mas, neste caso, era preciso saber o máximo de oxigênio que o respirador conseguiria entregar ao paciente, relata. Sabia-se que era necessário ser mais de 31%, então foi colocando dióxido de carbono ou nitrogênio em lugar do oxigênio. À medida em que eram colocados, proporcionalmente, era possível determinar o quanto de O2 estaria sendo entregue.

Criatividade
Segundo Krieger Filho, tal procedimento não se presta a uma análise para uma certificação do equipamento, por exemplo, mas mostrou-se útil para que a equipe responsável pelo ventilador pulmonar conseguisse ter uma estimativa laboratorial do teor de oxigênio presente na mistura de gases que sai do aparelho para o paciente. Busca-se um equipamento que consiga entregar algo próximo a 100% de oxigênio. O mais importante é que, ao final do dia, conseguimos ser úteis e prover uma medição que se mostrou muito próxima das estimativas teóricas acerca do equipamento, aponta. Se houvesse um aparelho que conseguisse medir a presença de oxigênio em até 100%, teria sido perfeito, mas não existia e o tempo era curto. Então, improvisou-se com criatividade.

Os testes incluíram a simulação de um reservatório para conectar a saída do respirador ao analisador de gases. Aqui também a criatividade contou positivamente. Foi utilizado um balão de festa de aniversário, uma bexiga. A ponta do medidor de vazão de oxigênio era mais ou menos do diâmetro de um lápis, e encaixava na boca da bexiga. Na parte de baixo do balão, fizemos um outro orifício, e então fomos aumentando o volume do balão, enchendo de ar, descreve Krieger Filho. Neste caso, o objetivo era ter medidas na frequência da respiração do paciente, para permitir a sincronização do fornecimento de oxigênio do aparelho com a frequência respiratória. Foram testadas várias frequências respiratórias, porque havia controle das variáveis, tais como pressão e vazão.

Segundo o professor, na corrida contra o tempo que caracteriza esse período de pandemia, o mais importante é conseguir prover ajuda real em tempo hábil. Tests on animals, humans and technical evaluations have shown respirator efficiency

Inspire, emergency lung ventilator developed at USP can be produced in up to two hours, being cheaper than the devices available on the market; while conventional pulmonary ventilators cost, on average, R $ 15 thousand, Inspire's value is R $ 1 thousand passed through the final stages of tests.

Animals and humans

Poli reported that in the last days 17, 18 and 19 April, studies were carried out with human patients, following the procedures of the National Research Ethics Commission. The essay, under the coordination of Professor José Otávio Auler Junior, also had the collaboration of Professor Filomena Galas, from the USP School of Medicine (FMUSP), and the physiotherapist Alcino Costa Leme. The gas lines (oxygen, nitrogen and carbon dioxide), as well as meters and gas analyzers in the laboratory to design a test that could indicate the oxygen content delivered by respirators. According to Professor Krieger Filho, as the laboratory meters are aimed at the analysis of combustion processes in vehicular engines and industrial burners, it was necessary to use creativity to adapt the conditions of the structure to the Inspire tests within the short time required for the task.

For example, the gas meter we have only measures the presence of oxygen in the gas mixture up to 31%; that's what it allows. But, in this case, it was necessary to know the maximum oxygen that the respirator would be able to deliver to the patient, he reports. It was known that it was necessary to be more than 31%, so it was putting carbon dioxide or nitrogen in place of oxygen. As they were placed, proportionally, it was possible to determine how much O2 was being delivered.

Creativity
According to Krieger Filho, this procedure is not suitable for an analysis for equipment certification, for example, but it proved to be useful for the team responsible for the pulmonary ventilator to be able to have a laboratory estimate of the oxygen content present in the gas mixture that leaves the device to the patient. We are looking for equipment that can deliver something close to 100% oxygen. The most important thing is that, at the end of the day, we managed to be useful and provide a measurement that proved to be very close to theoretical estimates about the equipment, he points out. If there was a device that could measure the presence of oxygen up to 100%, it would have been perfect, but it did not exist and time was short. So, he improvised with creativity.

Tests included simulating a reservoir to connect the respirator outlet to the gas analyzer. Here, too, creativity counted positively. A birthday party balloon, a bladder was used. The tip of the oxygen flow meter was about the diameter of a pencil, and fitted into the bladder mouth. At the bottom of the balloon, we made another hole, and then we increased the volume of the balloon, filling it with air, describes Krieger Filho. In this case, the objective was to have measurements on the patient's breathing rate, to allow the oxygen supply of the device to be synchronized with the respiratory rate. Various respiratory rates were tested, because there was control of variables, such as pressure and flow.

Art? Light pollution? A beacon of hope? A public relations action?

Since March 24, Swiss light artist Gerry Hofstetter has been projecting images onto the face of the outer Matterhorn, the iconic mountain located above Zermatt, Switzerland. The reasons vary: flags, hashtags, icons and short messages.

The small town of Zermatt asked if I would be willing to live up here in a camp for a few weeks during the coronavirus crisis. The light is a sign of hope that our team of six people is giving to the world, Hofstetter told swissinfo.ch via cell phone.

Arte? Poluição luminosa? Um farol de esperança? Uma ação de relações públicas? 

Desde 24 de março, o artista de luz suíço Gerry Hofstetter  externo está projetando imagens na face do Matterhorn externo, a icônica montanha localizada acima de Zermatt, na Suíça. Os motivos variam: bandeiras, hashtags, ícones e mensagens curtas.

A cidadezinha de Zermatt perguntou se eu estaria disposto a morar aqui em cima em um acampamento por algumas semanas durante a crise do coronavírus. A luz é um sinal de esperança que nossa equipe de seis pessoas está dando ao mundo, disse Hofstetter para swissinfo.ch via celular.
